Usually it is best if you need to create your own form layout or customize an existing to first copy the form, and then work with that copy to make your changes.
Follow these steps to do copy an existing form:
1. Go to the form layout designer (Tools > Form Layout Wizard)
2. Open the form you want to make a copy of
3. Then from the Form pull-down menu at the top choose Save As.
4. Now in the box labeled Name for the form, type your new form name. IMPORTANT: Be sure to make the form name unique within the first 5 characters. The form layout designer will sometimes not load forms properly if there are multiple forms with names that are very similar.
5. Then click OK, and you will now be working with the new copied form.
